This eliminates transport, improves continuity, and allows the care team to act on signals from monitoring and daily observation in real time.

Wells Health operates inside assisted living, memory care, skilled nursing, and CCRC settings. The program is also delivered to homebound seniors and across adult day and PACE settings where applicable.

From the first month, a community gains continuous on-site clinical presence, monitoring, and care coordination that most buildings cannot staff on their own. Residents are watched more closely, issues are caught earlier, and the community operates at a higher clinical standard. More than one hundred communities run the program today.

Wells Health is built around continuous presence inside the community, with clinicians, monitoring hardware, and care coordination operating as one program. The work is preventative throughout, focused on catching issues before they require acute intervention.
